Our practice offers an alternative to traditional dental insurance with our in-house savings plan. This care includes all preventive benefits of a traditional dental insurance plan, plus discounts on any recommended or elective treatment.*

*Only effective for 12 months from date of payment. Benefits do not rollover.

  • No yearly maximums
  • No pre-authorization requirements
  • No pre-existing condition limitations
  • No waiting periods
  • No insurance claims

Included In Your Plan

  • Two cleanings per year for Adult/Child. Four cleanings per year for Adult Perio.
  • Two clinical exams per year by Dr. Littlefield.
  • Standard radiographs once a year.
  • 15% off all other treatment and exams.**

**Does not apply to whitening services/products.

Premiums:

Total Annual Cost

Adult: $500
Adult Perio: $950
Child: $400

Program Guidelines

  • Patients agree that Littlefield Dentistry's fees must be paid tathe time of any services rendered.
  • Cannot be used in conjunction with another dental plan of any kind.
  • Non-refundable.
  • This is not an insurance product.

Program Exclusions & Limitations

This program is a membership plan, not a dental insurance plan. It cannot be used:

  • In conjunction with another dental plan, dental insurance, or financing program such a s CareCredit.
  • For treatment which, in the sole opinion of our doctor, lies outside of her scope.
  • For referrals ot specialists.
  • For hospitalization or hospital charges of any kind.
  • For costs of dental care which are covered under automobile/medical.
  • For services of injuries covered under workers' compensation.
